Final Fieldwork Verification Form (FFVF)

The end-of-fieldwork BACB® form submitted with your certification application, summarizing your total hours.

What it means

The Final Fieldwork Verification Form (F-FVF) is completed at the end of a fieldwork experience and submitted as part of your BCBA® certification application. On it, the supervisor attests that the trainee completed the fieldwork in compliance with all requirements. Like the monthly form, it must be retained for at least 7 years, and applicants after Jan 1, 2027 use the 2027 version.

Example

After finishing 2,000 hours across two supervisors, you submit one Final form per supervisor, per fieldwork type, with your application.

Where it trips people up

You complete one Final form per supervisor, per fieldwork type — not a single form for everything. If a supervisor won’t sign despite the contract terms being met, the BACB® has a Contested Fieldwork process.
