2027 Monthly Fieldwork Verification Form revision for BCBA trainees: which version to use

If you are working toward BCBA certification and look at the footers of your monthly verification forms, you may find two different versions. In June 2026 the BACB replaced both 2027 Monthly Fieldwork Verification Forms, the Individual Supervisor form and the Multiple Supervisors at One Organization form. The older file says Version 02/2026 in the footer. The new one says Version 06/2026.

The update came without an announcement, so many trainees, supervisors, and programs are still working from saved copies of the older file. That is less of a problem than it sounds.

Any 2027 version is accepted

The BACB has confirmed that any version of the monthly form may be accepted, as long as it is for the 2027 Fieldwork Requirements. A month signed on 02/2026 does not need to be redone because a newer file exists.

That fits the rule in the BACB’s Fieldwork Checklist and Tip Sheet:

“You must use the M-FVF that meets the requirements at the time the application is submitted.”

What matters is the set of requirements a form belongs to, not its revision date. A 2022 form for a 2027 application is the real problem, and moving those months onto 2027 forms is covered in Applying for BCBA in 2027? Qualifying hours on 2022 forms need to be transferred to 2027 forms.

What changed in June

Nothing about the requirements, and none of the wording. Both versions have the same fields and the same attestations. The changes are in the layout and, on one form, in how the totals work.

The totals moved. On the 02/2026 forms, Total Fieldwork Hours and Percent of Hours Supervised sit at the top right of the hours section. On 06/2026 they sit below Independent Hours and Supervised Hours, and the signature lines moved down the page to make room.

The Individual form’s totals now fill in. On the 02/2026 Individual Supervisor form, the total hours, total minutes, and percentage boxes are locked, and the calculations meant to fill them are in the file but not connected to the boxes. Filled in on screen, those three boxes stay empty and cannot be typed into. On 06/2026 they calculate automatically from the hours you enter.

The Multiple Supervisors at One Organization form calculated its totals on both versions. For that form, the June update is layout only.

The one thing to check on older forms

If you filled in 02/2026 Individual forms on screen, open a few and look at the total and percentage boxes. They may be empty.

The version is accepted, but an empty box is a separate matter: the form asks for those numbers. If you find months like this, raise it with your supervisor. Forms that were printed and completed by hand are not affected, since the boxes were only locked on screen.

What to do now

  1. Use a 2027 form for every month you count under the 2027 BCBA requirements. Either 02/2026 or 06/2026 is accepted.
  2. For new months, use 06/2026. It is the current file, and its totals fill in for you.
  3. Check your 02/2026 Individual forms for empty total and percentage boxes, and talk to your supervisor if you find any.
